Complete this form to request up to two (2) pregnancy test. Pregnancy test are only offered individually, not for distribution by offices or departments. It is mandatory for you or a representative party to come to Nebraska Union, Room 340 to retrieve your test. You will receive an email from Gender and Sexuality Center's staff when your order is ready to be picked up. Thank you.
